    Tentative start date of the grant will be September - October 2018

    First 6 months – planning phase (some involvement will be needed from the evaluation team to work on evaluation plan, logic models, identifying measures, instruments for data collection etc.)

    This is a 3-year project – so tentative end date will be Sept-Oct 2021


    We have earmarked about $80,000 for the project evaluation for the entire grant period (with bulk of the work being done in the 2nd and 3rd year).

    Summary/Abstract:

    The goal of the Dane County (Wisconsin) Adult Community Services (ACS) Division, Department of Human Services’ Alzheimer’s Disease and Related Dementia (ADRD) grant is to strengthen our existing dementia capable system of specialized supportive services so persons with dementia can live in their homes for as long as possible.  The objectives of our project, evidence-based/evidence informed strategies, and anticipated outcomes are as follows:

    1.                         Care manage and deliver supportive services to persons living alone with or at risk for Dementia in 4 housing complexes for low-income, disabled and elderly, using Gatekeeper identification, ADRD screening, Dementia Support Team Wraparound Intervention using ethnographic care mapping, and home and community-based supports (HCBS) to delay restrictive placement, reduce emergency responder, ER visits and hospital admissions, and reduce unmet needs for persons living alone; 

    2.                         Improve the quality and effectiveness of HCBS’s for individuals living with IDD and at risk for developing dementia through the development and dissemination of specific training modules for providers to better serve this population and individual nurse consultations, as measured by increased competency of HCBS staff in provision of service to this unique population, and improved retention of staff who receive specialized training throughout the course of the grant;

    3.     Provide behavioral symptom management training and expert consultation to family

    caregivers through expanded Dementia Crisis Specialist/Dementia Care Specialist services to stabilize in place more persons experiencing a dementia related crisis thereby by reducing trauma and caregiver stress, reliance on first responders when managing challenging behaviors, and out of home placement into more restrictive environments.

    The Wisconsin Supplemental Nutrition Program for Women, Infants, and Children (WIC) is evaluating program caseload and developing strategies to improve participant retention. One of these strategies is to conduct focus groups with current and past WIC participants around the state.

    Wisconsin WIC is in need of facilitator/evaluator to conduct focus groups within the following timeline:

    • Facilitate aworkgroup call no later than June 29, 2018
    • Conduct three English focus groups and one Spanish focus group in different locations in Wisconsin no later than August 31, 2018
    • Submit data analysis and interpretation to Wisconsin WIC no later than September 30, 2018
    • Provide written summary to Wisconsin WIC no later than September 30, 2018

    Evaluators: Attached and below is the information for Fellow applications for this year’s CDC Evaluation Fellowship Program. Please circulate this widely to people or organizations that might get us to good Fellow applicants. Application deadline for Fellows is April 13th, with Fellows starting in mid-August. See attachment for full info.

    The Centers for Disease Control and Prevention (CDC) seeks applicants for the 2018 class of the CDC Evaluation Fellowship Program. This is the 8th year of this initiative, and represents a major commitment by CDC to program evaluation and program improvement. Fellows work under the leadership of CDC's Chief Evaluation Officer; they will be matched with CDC host programs in Atlanta, Georgia to work on program evaluation activities for/with those programs.

    About the Fellowship

    · The CDC Evaluation Fellowship Program aims to expand the capacity of CDC programs to conduct evaluation and increase its usefulness and impact.

    · The Evaluation Fellowship Program signifies CDC’s dual commitment to making program evaluation a standard part of practice and to developing a cadre of professionals with the skills to make that happen. With the CDC Evaluation Fellowship, programs will have the resources, tools, and leadership to continuously improve their work.

    · Started in 2011 with an initial class of 5 Fellows, the Fellowship has about 15-20 Fellows each year.

    · Though the Fellowship has grown dramatically in just a few years, the Fellowship remains highly competitive, with only about 10% of applicants being selected for the Evaluation Fellowship Program.

    · The Fellowship is intended to be a two-year program, with the second year contingent on satisfactory performance and availability of funds.

    About the CDC Evaluation Fellows

    · Experience: Fellows are PhD or master’s degree professionals with backgrounds in evaluation, behavioral and social sciences, public health, and other disciplines relevant to CDC’s work. Successful applicants also typically have significant experience in applied evaluation projects.

    · Placement: Fellows are based in Atlanta working within one of CDC’s programs.

    · Funding: Fellows receive a monthly stipend depending on education level and experience and, unlike many fellowships; CDC Evaluation Fellows receive substantial financial support for their training and professional development, and a supplement to offset health insurance costs.

    Applying for the Fellowship

    · Applicants for the Fellowship go through an extensive selection process, including interviews with potential host programs, culminating in CDC host programs and Fellows matched based on mutual interest.

    · Candidates must have received their qualifying degree within the past 5 years.

    · Qualifying degree must be completed by end of Spring semester of the year candidate is applying and on transcript by June.

    · Fellows are brought on through the Oak Ridge Institute for Science and Education (ORISE) and are not considered full time employees of CDC during the fellowship.

    · This is a training fellowship so Fellows do not become employees of CDC.

    Responsible for coordination, development, and implementation of quality assurances, improvement systems, compliance, and client satisfaction and program evaluation for the Social Services program.

    Responsibilities:

    1. Audit files, provider agreements, administrative codes, polices, procedures, etc. to ensure all legal, organizational, and contractual requirements are fulfilled. E
    2. Research, develop and assist with implementation of polices and procedures that will ensure compliance. E
    3. Work closely with Compliance Coordinator to understand contract requirements and recommend, develop and implement plans to meet all requirements. E
    4. Evaluate program performance, recommend improvements, and improve quality of services by using information collected via various sources (participants, providers, etc.) and methods (surveys, questionnaires, etc.) E
    5. Provide recommendation and consultation to all members of the leadearship team. E
    6. Complete and submit required items by due dates; to include, but not limited to: reports, program evaluations, personnel items, etc. E
    7. Follow legal, organizational and contractual requirements, laws and policies. E
    8. Establish and maintain regular good communication, collaboration, and cooperation with all stakeholders (i.e., staff, providers, etc.). E
    9. Implement and/or practice the duties and responsibilities of a mandated reporter of suspected child abuse under current policies and procedures, as required. E
    10. Attend all meetings and professional development activities, as directed. NE

    Qualifications:

    1. Bachelor Degree from an accredited school in Business Management or related field.
    2. Minimum two (2) years experience in Quality Assurance or Quality Inspection in a program setting.
    3. Able to work flexible hours, including evenings and weekends depending on needs of the program.
    4. Must successfully complete and pass all background checks.
    5. Strong cultural competency and interpersonal relationship skills.
    6. Strong communication skills, written and verbal, appropriate to target audiences across multiple functional areas, categories, levels, linguistic, and economic backgrounds; both in a team and individual environment.
    7. Excellent critical thinking; able to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
    8. Highly developed organizational skills and able to manage multiple priorities.
    9. Knowledge and experience with Microsoft Office suite.
    10. Transportation, valid Wisconsin Driver's license, state minimum auto insurance, and must meet La Causa, Inc. driving standards.

    Evaluation brownbag, next Monday the 27th at Noon at 445 Henry Mall, in basement conference room B8.  The topic will be informal debriefing (i.e. interesting things we took away, people we met etc.) from the American Evaluation Association (AEA) conference.
